Use the form below to upload a copy of your prescription for our files. We can accept most file formats including JPG, PDF, etc. Once we have your prescription on file we'll add it to your records for reference on existing and future orders.
Once your prescription has been submitted our Service Team will review the details and contact you with any questions. If you have an order on hold waiting for a prescription we will ship it immediately once a valid prescription is received.
